NHS survey aimed at people living with dementia and their carers

NHS England is gathering information about dementia services in the West Midlands and is asking people to help by completing a survey.

There is one set of questions for people living with dementia and another for carers.

NHS England wants to find out about any services that people find helpful and any that they think are missing.

The survey is also asking about any problems people have had finding out about, or getting, the support they feel they need.
