Exploring ideas to support patients through hospital discharge and after care

Dudley CVS and Healthwatch Dudley are hosting these sessions to bring together a range of providers from the voluntary, private and statutory sectors to explore the potential role they can play in supporting people in hospital and in their own homes following discharge.

The objectives are to:

Learn about what came out of the first session held in December

Learn about the hospital discharge pathway

Learn from Dudley Group and Adult Social Care about what is working really well currently and what is not across health and social care

Explore where providers could add value

Decide on ideas, options for funding and putting together proposals

This session is being repeated three times to engage a wide range of providers. Participants only need to attend one session.

The sessions are being held on the following dates:

Wednesday June 10, 10am – 12:30pm in Brierley Hill. Book through Eventbrite here
